- The company’s name, address and department – so we can locate the workplace in case of a visit
- What the health and safety issue relates to
- How many people are affected by the issue
- When the issue began and how long it is expected to last
- Whether the company has a health and safety organisation and whether it has been involved
- Your name and telephone number – in case we need further information.
